Key Job Details

• Must be licensed to practice Physical Therapy in Oklahoma.
• Weekend coverage required: at least 1 Saturday per month, with flexibility for additional weekend shifts.
• Acute inpatient care setting.

Required Credentials & Documentation

Licensure & Certifications

• Valid Oklahoma PT license (Primary Source Verification required).
• Basic Life Support (BLS) - AHA or ARC only, required for all clinical positions.
• Advanced Cardiac Life Support (ACLS) - AHA or ARC only, required for ICU, ER, L&D, Cath Lab Tech, RRT.
• Pediatric Advanced Life Support (PALS) - AHA or ARC, required for ER.
• Neonatal Resuscitation Program (NRP) - required for L&D, NICU, RRT.
• National certification verification as applicable (e.g., Allied Licensure Report for non-RN/LPN roles).

Background Checks

• 7-year criminal background check within 30 days of start, updated annually.
• Must include all locations lived/worked in the past 7 years.
• Checks include Social Security Trace, Counties, Nationwide, Sex Offender, FACIS Level 3.
• EPLS/GSA/SAM and HHS/OIG checks required.

Health & Immunization Requirements

• Physical exam within 1 year of start.
• Tetanus/Diphtheria/Tdap vaccine within 10 years (declination acceptable).
• Negative TB test (2-step PPD, T-spot, or Q-Gold) within 12 months; positive results require chest X-ray within 1 year.
• Measles (Rubeola), Mumps, Rubella, Varicella: Positive titer or proof of 2 vaccines required; no declinations/exemptions accepted.
• Hepatitis B: Positive titer, 3-dose vaccine series, or declination required.
• Flu vaccine documentation required during flu season (Oct 1 - Mar 31); declinations accepted with proper waiver.
• COVID-19 vaccine: New Comirnaty mRNA 2024-2025 booster required or facility-specific medical/religious waiver.

Drug Testing

• 10-panel drug screen within 30 days of start; instant drug screens accepted.
• Results must be finalized prior to start date.

Competency & Orientation

• Medication Test/Pharmacology Competency Exam as applicable.
• General Competency Exam as applicable.
• Completion of facility-specific orientation modules (approx. 2-3 hours) prior to start.
• Review and sign Commitment to Excellence Standards and Confidentiality agreements.
• Photo for badge required (passport-style with solid background).

Additional Requirements

• Government-issued photo ID (Driver’s License or State ID).
• I9 or attestation required.
• E-Verify or attestation required.
• Residency Declaration Form if working on a compact license.
• Employment application and skills checklist (updated annually).
• Two professional references within 3 years of start.

Notes

• Orientation includes a fit mask test and color-blind exam conducted onsite.
• Facility-specific forms for parking, job description acknowledgment, and vaccine waivers must be completed prior to start.
• HealthStream modules must be completed by Friday afternoon before a Monday start date.
