Job Description

Seeking a Stroke Coordinator to assist with survey readiness.

Shift Details:

• 5 days a week, 8-hour shifts (0800-1630)
• Monday through Friday
• No floating, on-call, or weekend requirements

Required Qualifications

• Valid RN license in California
• Minimum 2 years of experience as a Stroke Coordinator, including experience within the last year
• BLS certification issued by the American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Certifications (as applicable)

• ACLS (Advanced Cardiac Life Support) – AHA or ARC preferred
• Other certifications may be required based on position specifics (verify as needed)

Background and Compliance Requirements

• 7-year background check required within 6 months of start
• Annual updates for EPLS/GSA/SAM, HHS/OIG, and FACIS Level 3 screening
• Education verification (Bachelor of Science in Nursing preferred)
• Sex offender search within 90 days of start
• Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per California law)
• Physical exam within 1 year of start
• Fit mask test within 1 year of start, renewed annually
• Immunizations and health screenings including:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest x-ray within 90 days of start, renewed annually
  • Measles, Mumps, Rubella, Hepatitis B, Varicella immunity or vaccination documentation
  • Seasonal flu vaccination during flu season (Nov-April) or signed declination
• Respiratory safety clearance prior to start, renewed annually
• References: 2 professional references within 3 years (Aya requirement)
• Full 7 years of work history required on application with no gaps

Additional Notes

• Candidates must review and sign various facility-specific documents prior to start, including standards of business conduct, code of conduct, and vendor training.
• Religious exemptions for vaccines and facial hair (PAPR) may be considered with appropriate documentation and approval.
