Licensed Practical Nurse (LPN) - Travel/Contract

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and attend periodic office meetings.
• Manage an average of 5-8 patients per day (7-8 patients daily expected).
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver’s license, and required liability insurance.
• Schedule: Monday to Friday, 8 AM to 5 PM, with at least one weekend per month on call.
• Patient visits may include both home and care facility settings.
• Must be willing to travel up to 75 miles from the home clinic.

Required Qualifications

• Valid LPN/LVN license in Georgia (license must be current and in hand; no pending licenses accepted).
• Minimum of 1 year of recent Home Health experience within the last year, including experience working with adult patients.
• Hospice experience is required.
• Completion of at least one travel assignment.
• Current BLS certification issued by the American Heart Association or American Red Cross.
• Experience in a Home Health setting is mandatory.

Productivity Expectations

• Approximately 35 patients per week.
• Expected to meet 25-30 weekly units of productivity.

Additional Requirements & Compliance

• Must complete a standard background check including 7 years of history, sex offender search, and drug screening.
• Required health documents include: Physical exam within 12 months, negative TB test or chest X-ray, immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella, and COVID vaccination or declination.
• Must have personal or auto insurance policy under candidate’s name or list candidate as a driver.
• Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start.
• Must complete all state-specific background and fingerprinting requirements as applicable.
• Two professional references required.
• Work history verification for at least the last 7 years or two most recent employers.

Travel Experience

• Prior travel experience is required; candidate must have completed at least one travel assignment.

Notes

• This is a true contract assignment with potential for hire at the end of the contract.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
• Local candidates are accepted under the same terms as travelers.
