Job Details

• Specialty: Hospice
• Schedule: Monday to Friday, with support required one weekend per week
• Average Patients per Day: 5-7 (Geriatric cases)
• Guaranteed Hours: 40 hours per week
• EMR System: Experience required with HCHB
• Minimum Specialty Experience: 1 year, recent
• Weekly Points Requirement: Minimum of 30 points weekly
• On-Call: Yes (details to be discussed during interview)
• Travel Radius: Up to 60 miles from the hub/office
• Locals Accepted: Yes (at travel rate)
• Dress Code: Scrubs (any color), no jeans
• Device Provided: iPad provided; cell phone use not allowed
• Mileage Reimbursement: Yes, at IRS standard rate

Required Qualifications

• Minimum of 1 year of hospice nursing experience within the last year
• Current Registered Nurse (RN) license in hand for Washington state
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
• Experience with Homecare Homebase (HCHB) EMR system

Background and Compliance Requirements

• Background checks including SSN trace, county searches (last 7 years), sex offender registry, motor vehicle records, education and work history verification
• EPLS/GSA/SAM, HHS/OIG checks required within 90 days of start
• Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
• Education verification of highest degree related to profession
• State-specific criminal history and employability status checks as applicable
• Signed orientation safety manual acknowledgment (updated annually)
• Primary source verification of state license and NURSYS verification within 90 days of start
• Medication/pharmacology competency exam as applicable
• Agency background attestation required

Health and Safety Requirements

• Physical exam within the last 12 months (not required annually)
• Fit test for N95 mask (no specific model required)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
• Annual TB questionnaire required
• Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella (vaccination, titer, or declination accepted)
• 9-panel drug screen within 12 months (instant acceptable)
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Additional Requirements

• Two professional references within 2 years of start (travel, allied, per diem acute care)
• Full 7 years of work history documented with no gaps
• Annual skills checklist
• Proof of current auto insurance if job responsibilities include driving
