Location: Wenatchee, Washington
Assignment Length: 6 months
Shift: Day shift, 5 days a week, 8 hours per day (9:00 AM - 5:00 PM)
Unit: Skilled Nursing Facility (SNF)

Job Summary

Seeking a Physical Therapist Assistant (PTA) for a travel contract position in a Skilled Nursing Facility. The role involves providing therapy and rehabilitation services in a post-acute care setting.

Required Qualifications

Valid Physical Therapy Assistant license in Washington
Minimum 1 year of Physical Therapy Assistant experience within the last year

Compliance and Documentation Requirements

Government-issued photo ID (Driver’s License or State ID)
Emergency Contact Form (required prior to start)
Background checks within 1 year of start, including SS Trace, County searches (7 years), Nationwide, and Sex Offender registry
Annual EPLS/GSA/SAM and HHS/OIG checks
Physical exam within 1 year of start
Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest X-ray within 1 year
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
9-panel drug screen within 1 year of start (instant drug screen acceptable)
Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
Annual skills checklist
Professional references:
- 2 for travel, allied, or per diem acute care roles
- 1 for per diem non-acute care roles

Facility located along the Columbia River, approximately 24 miles from Leavenworth, WA
