Job Opportunity: Home Health Registered Nurse
Location: Youngstown, Ohio
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Specialty: Home Health Nursing
- Minimum Experience Required: 1 year recent experience in specialty
- Average Patients per Day: 5-7
- Weekly Hours: Minimum 30 points weekly, guaranteed 40 hours
- Types of Cases: Geriatric patients
- First-time travelers accepted
- On-call Requirement: Yes, every 3rd to 4th weekend
- Territory: Up to a 60-mile radius from the hub/office
- Locals accepted at travel rate
- Dress Code: Scrubs (any color), no jeans
- Documentation and communication must be timely and accurate
- iPad provided for work; cell phone use not allowed during work
- Mileage reimbursed at IRS standard rate
- Flexibility with territory expected
Required Qualifications
- Valid Registered Nurse (RN) license in Ohio
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum of 1 year recent experience in Home Health nursing, including experience within the last year
Background and Compliance Requirements
- Background checks required within 90 days of start and updated annually, including:
- SSN Trace
- County searches for last 7 years of residence/work/school
- FACIS Level 1
- Nationwide Sex Offender Registry
- Motor Vehicle Records (MVR)
- Education and Work History Verification
- State-specific searches as applicable
- Fingerprinting required (Ohio BCI and FBI) within 12 months of start and updated annually
- Education verification of highest degree related to nursing
- NURSYS verification for RN license
- Medication and competency exams as applicable
- Signed acknowledgment of Orientation Safety Manual (updated annually)
- Proof of current auto insurance required due to driving responsibilities
- Facility-specific policies and attestations must be completed prior to start
Health and Immunization Requirements
- Physical exam within last 12 months (physician statement)
- Negative TB test or chest X-ray within required timeframes
- Annual TB questionnaire
- Immunizations or titers for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Drug screening (9 panel) within last 12 months
- Seasonal flu vaccination or facility-specific declination form during flu season
- COVID vaccination or facility-specific declination
Additional Requirements
- 2 professional references within 2 years of start
- Full 7 years of work history documented with no gaps
- Annual skills checklist and work history verification
- Must comply with facility’s acceptable use policy and other onboarding documents
