Speech-Language Pathologist (SLP) - Skilled Nursing Facility (SNF) Travel/Contract Position

Assignment Details

Start Date: ASAP
Assignment Length: 13 to 26 weeks preferred
Weekly Hours: 36 to 40 hours
Shift: Day shift, 5 days per week, 8-hour shifts (8:00 AM to 4:30 PM)
May include occasional weekend coverage and floating as needed
Cancellation Notice: 14 days

Requirements/Qualifications

Valid Speech Language Pathologist license in California
Minimum 1 year of Speech Language Pathologist experience
Speech Language Pathologist experience within the last year

Required Certifications and Licenses

Current state license for Speech-Language Pathology
BLS (Basic Life Support) certification from American Heart Association or American Red Cross
ACLS (Advanced Cardiac Life Support) certification preferred
PALS (Pediatric Advanced Life Support) certification preferred

Required Documentation and Compliance

Government-issued photo ID (Driver’s License or State ID)
Emergency Contact Form (prior to start)
Background checks including:
- Standard 7-year search (locations lived/worked/school)
- EPLS/GSA/SAM (annual)
- HHS/OIG (annual)
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella (Chickenpox)
9 Panel Drug Screen within 1 year of start
Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
Completion of facility-specific compliance quiz prior to start
Medication Test/Pharmacology Competency Exam as applicable
