Job Opportunity: PT Home Health (Therapy/Rehabilitation)

Job Description

• Specialty: Home Health Physical Therapy
• Territory: All Suffolk area, up to a 60-mile radius from the hub/office
• Average Patients per Day: 5-7 (to be confirmed during interview)
• Types of Cases: Geriatric
• Guaranteed Hours: 40 hours/week
• First-time travelers accepted
• Locals accepted at travel rate
• Dress Code: Scrubs (any color), no jeans
• Device Provided: iPad (cell phone not allowed)
• Mileage reimbursed at IRS standard rate
• Flexibility with territory expected
• Timely documentation and communication required

Required Qualifications

• Valid Physical Therapist license in Virginia
• Minimum 1 year of recent Physical Therapist experience within the last year
• Experience in a Home Health setting
• Experience working with Geriatric patients
• BLS (Basic Life Support) certification required (AHA, ARC, Military acceptable)
• Experience with OASIS documentation
• Experience with Homecare Homebase (HCHB) EMR
• No on-call required

Compliance and Background Requirements

• Background checks required within 90 days of start, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1 M
  • Nationwide Sex Offender
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification
  • State-specific searches (VA State Police Search, Kentucky Elder Abuse Registry, Indiana Statewide Search, etc.)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting may be required depending on state placement
• Education verification of highest degree related to profession
• NURSYS verification (for PT license verification if applicable)
• Medication/Pharmacology competency exam as applicable
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Personal or Auto Insurance proof required (for Home Health driving responsibilities)
• Facility-specific policies and attestations must be completed prior to start

Health and Immunization Requirements

• Physical exam within last 12 months (physician statement)
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire required
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9 Panel Drug Screen within last 12 months
• Flu seasonal vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Documentation and History

• 2 professional references within 2 years of start (travel, allied, per diem acute care)
• Full 7 years of work history documented with no gaps
• Work history verification within 90 days of start
• Annual skills checklist

• Candidates must be flexible with travel territory within the specified radius
• Timely and accurate documentation is critical for this role
• Scrubs required; no jeans allowed on assignment
