Registered Nurse - Case Manager
Location: Atlanta, Georgia Employment Type: Travel/Contract Contract Length: 12 weeks Shift: Day (5x8 hours), 8:00 to 16:30 Hours: 8 per day, 40 per week
Role Details
- Schedule: Monday to Friday. No weekends. No call.
- Floating: Within facility and scope of practice as needed.
- Responsibilities:
- Assess and evaluate patients, and drive the discharge plan
- Coordinate services to the patient's appropriate level of care
- Initiate, evaluate, and re-evaluate the treatment plan daily
- Monitor progress daily and identify barriers to discharge
- Monitor length of stay
- Follow Medicare regulatory guidelines on patient choice, IMM notices, and MOONS
- Clinical Screen: Reviewed by the clinical team for fit. If the candidate passes, the manager makes the final decision. Screens are handled case by case.
- Time Off: Maximum 7 days of RTO. Discuss requests during the interview. The manager has asked for no RTO during September.
- Holidays: Travelers must work one major holiday if one falls in the assignment. If two fall, they must work both. If three fall, they must work two. Major holidays are Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Year's Eve/New Year's Day.
- Orientation: Non-billable orientation hours equal the contract length in weeks (for example, 8 hours for an 8-week assignment). Full compliance is required to start on time. Otherwise, the start is pushed back 2 weeks due to the bi-weekly orientation schedule.
Required Qualifications
- Valid RN license in Georgia
- Associate of Science in Nursing / Associate Degree in Nursing
- Case Management experience (3 years)
- Case Management experience within the last year
- One completed travel assignment
