Travel/Contract Physical Therapist (Pediatric Rehab)

Location: Colchester, Vermont

Job Description

• Position on Pediatric Rehab Team providing home and childcare visits.
• Schedule: Monday to Friday, no weekends.
• Patient population includes children with:

  • Autism Spectrum Disorder (ASD)
  • Down Syndrome
  • Developmental delays
  • Torticollis, plagiocephaly
  • Cerebral palsy (CP), hypoxic-ischemic encephalopathy (HIE), neuro microcephaly, hydrocephaly
  • Orthopedic conditions affecting home mobility (acute)
  • Club foot, prematurity, brachial plexus birth injury
• Distance measured as straight line, not driving distance.

Requirements

• Licensed Physical Therapist with Vermont state license.
• Minimum of 2 years clinical/specialty experience in Physical Therapy.
• Prior travel experience NOT required.
• Experience with Epic electronic medical records system within the past 24 months.
• Reliable personal or rental vehicle strongly recommended (public transportation not accessible).
• Vehicle must have all-season or snow tires during November through April.

Certifications & Verifications

• ACLS (Advanced Cardiac Life Support) - AHA certified
• BLS (Basic Life Support) - AHA certified
• PALS (Pediatric Advanced Life Support) - AHA certified
• State license primary source verification within 30 days of start and upon expiration
• Motor Vehicle Report (MVR) release required
• Background check within 30 days of start covering all locations lived/worked/schooled in past 7 years, updated annually
• Education verification as applicable
• National tech certification verification within 30 days of assignment start
• Orientation Safety Manual Acknowledgement (annual)
• Medication/Pharmacology competency exam as required
• Color blind testing online as required
• Competency exams as required by specialty

Health & Immunization Requirements

• Physical exam within 12 months of start
• N95 Fit Mask test (annual)
• Tdap vaccination within 10 years or titer
• Negative TB test (PPD, QGold, or TSPOT) within 12 months; chest X-ray if positive TB test
• Annual TB questionnaire if positive TB
• 2 MMR vaccines or positive titers for Rubeola (Measles), Mumps, Rubella
• Hepatitis B series and titer or signed declination
• 2 Varicella vaccines or positive titer
• Flu vaccine or exemption required during flu season (Oct 1 - Mar 31)
• COVID-19 vaccination or declination required
• Pneumonia vaccination or declination if assigned to NY facility

Additional Requirements & Notes

• Complete all work history details including bed count, EMR, and facility type for submission.
• Employment gaps over 90 days must be explained.
• Two professional references from assignments within the last 3 years required.
• Skills checklist within 12 months required.
• 2-year verification of most recent clinical work history required.
• Auto insurance required and must be valid throughout assignment.
• Must complete EPIC and Cornerstone training modules prior to start (approx. 8 hours, non-billable).
• Must complete Vermont Adult and Child Abuse Registry Consent Form.
• COVID testing protocol on Day 1 of work may be required.
• Must comply with dress code and telephone usage policies.
• Photo for badge required (passport style, plain background).
• Auto registration form must be completed within 48 hours of signed contract.

Transportation & Location Notes

• Public transportation is not available; reliable personal or rental vehicle is necessary.
• Vehicle must be equipped with all-season or snow tires from November through April.
