Travel/Contract Physical Therapist (Therapy/Rehabilitation)

Location: Colchester, Vermont
Contract Length: 20 weeks

Job Description

• Adult Home Health Physical Therapist role
• Assignment duration: 16-32 weeks
• No 10-hour shifts; weekend rotations required
• Expected to see 5-6 clients per day
• Strong clinical and assessment skills required
• Must be fluent in navigating different EMRs; Epic experience is required
• Reliable personal or rental vehicle required (public transportation not accessible)
• Vehicles should have all-season or snow tires from November through April

Experience Requirements

• Licensed Physical Therapist (PT)
• Minimum of 3 years clinical experience in home health (or more if indicated elsewhere)
• Prior travel experience required; at least 2 previous travel assignments preferred

Certifications & Licenses

• Current State License verification required within 30 days of start
• ACLS, BLS, and PALS certifications as applicable (AHA standards)
• National Tech Certification verification within 30 days of assignment start
• Background check within 30 days of start, covering all locations lived/worked/schooled in past 7 years
• Motor Vehicle Report (MVR) required for Home Health and Hospice specialties
• Education verification as required

Orientation & Scheduling

• Bi-weekly orientations on Mondays (next available 12/01)
• No block schedules allowed
• Monday to Sunday work schedule

Health & Immunization Requirements

• Physical exam within 12 months of start
• N95 Fit Mask test required prior to start and updated annually
• Tdap vaccination within 10 years or titer
• Negative TB test or chest X-ray within past 12 months (updated annually)
• MMR (Rubeola, Mumps, Rubella) - 2 vaccines or positive titer
• Varicella - 2 vaccines or positive titer
• Hepatitis B series and titer or signed declination
• Seasonal flu vaccination or approved exemption required during flu season (10/1 – 3/31)
• COVID-19 vaccination or declination required
• Pneumonia vaccination or declination required if assigned to NY facility

Background & Compliance

• Vermont Adult and Child Abuse Registry consent and search required
• Vermont Crime Information Center (VCIC) criminal background check required for Vermont assignments
• COVID questionnaire and attestation required prior to start
• Drug testing as required (not annually for VT facilities)

Documentation & Other Requirements

• Complete and detailed work history including bed count, EMR, and facility type
• Explanation required for any employment gaps over 90 days
• 2 professional references from assignments within last 3 years
• Skills checklist within 12 months
• Employment application required
• Auto insurance proof required and must be valid throughout assignment
• Photo for badge (passport style, plain background, no logos or hats)
• Completion of EPIC and Cornerstone modules prior to start (approx. 8 hours, non-billable)
• Review of dress code and telephone usage policies prior to start
• Completion of emergency contact and other facility-specific forms

Additional Notes

• Reliable transportation is critical due to lack of public transit
• Vehicle must be equipped for winter conditions during November-April
• Candidate must comply with all facility-specific policies and documentation deadlines
