COVID-19 Vaccine: Required for all travelers (medical or religious exemptions accepted with documentation)
Indiana RN Restriction (Effective 11/30/24): Indiana RNs are generally not allowed to work at these facilities after this date, except for Perioperative/OR RNs, Allied, and Ambulatory RN travelers (including ENDO, IR RN, PACU, Cath Lab RN, SDS).

Experience Requirements

No explicit years of specialty experience stated; assume standard RN qualifications.
No prior travel experience required.

Required Certifications

BLS (Basic Life Support) – AHA only, RQI acceptable
ACLS (Advanced Cardiac Life Support) – AHA only, RQI acceptable
PALS (Pediatric Advanced Life Support) – AHA only, RQI acceptable
NRP (Neonatal Resuscitation Program) – AHA only
Other certifications as applicable per unit: CHEMO, TNCC, EFM, NIH Stroke Certification, Tech Cert Verification (as required)

Required Documents & Verifications

Valid Driver’s License or Passport (clear copy)
Background Check (within 90 days of start) including:
- 7 years residence/work history
- SSN Trace
- County Criminal, Statewide, National Sex Offender, FACIS Level III
- Education Verification
- International search if applicable
EPLS/GSA/SAM and HHS/OIG checks (within 30 days of start)
Transcript or Diploma (official, clear copy)
Education Verification (third party, may be pending at start)
State License Verification (within 30 days of start, primary source)
Physical Exam (within 12 months of start)
Fit Mask Test (within 12 months of start) with approved respirators
Negative 10 Panel Drug Screen (within 12 months of start)
Immunizations and Titers:
- COVID-19 vaccine (required)
- Flu Seasonal Vaccination or approved medical/religious exemption (required during flu season)
- Tetanus/Diphtheria/Tdap (preferred but not required)
- Tuberculosis test (PPD, QuantiFERON-TB Gold, or T-SPOT within 1 year)
- Rubeola (Measles), Mumps, Rubella, Varicella – Positive titer or 2 vaccines required; no declinations accepted
- Hepatitis B series, titer, or signed declination required
Drug Testing: Negative 10 Panel Drug Screen (Amphetamines, Marijuana, Cocaine, Opiates, PCP)
Color Blind Testing (Ishihara exam or equivalent, 100% minimum score)
Medication Test/Pharmacology Competency Exam
Unit Competency Exam

Facility-Specific Requirements

Completion of facility-specific forms and packets (Look of Assurance, Compliance Checklist, Confidentiality Agreement, Policy Information, Self Study Orientation & Test, Fast Track Slide Packet and Signature Forms)
References: Minimum 2 references from recent employment verified within 2 years; references must be from charge, manager, supervisor, or above.
Employment Application: Current with full work history; gaps of 30+ days must be explained.
Skills Checklist: Required within 12 months of start and annually.
TB Questionnaire: Required and must be updated annually.

This position requires compliance with strict immunization and documentation policies, including mandatory COVID-19 vaccination and flu vaccination or approved exemptions. Prior travel experience is not required, and first-time travelers are welcome. The contract length is approximately 13 weeks with 12-hour shift options.
