  • Background checks including 7-year history, EPLS/GSA/SAM, HHS/OIG (annual)
  • Certifications: ACLS, BLS, PALS (preferred), State License verification
  • Competency exams as applicable (Medication Test/Pharmacology, general competency)
  • Physical exam within 1 year of start
  • Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
  • Immunizations or proof of immunity for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug testing: 9-panel drug screen within 1 year of start
  • Seasonal flu vaccination or facility-specific declination during flu season (10/1 - 3/31)
  • COVID-19: Must be fully vaccinated plus booster prior to start; exemptions possible with approval and facility-specific forms
