  • Any call-outs must be given with more than 24 hours' notice

Health & Compliance Requirements

  • Physical Exam: Within 1 year of start
  • TB Test: Negative PPD, Quantiferon Gold, or T-spot within 1 year of start; if past positive, negative chest x-ray within 1 year required
  • Drug Testing: 9-panel drug screen within 1 year of start
  • Immunizations:
    • Measles (Rubeola), Mumps, Rubella: Proof of immunity or vaccination required for candidates born in 1957 or later
    • Hepatitis B: Titer, declination, or vaccine series required
    • Varicella: Titer, vaccination, or history of chickenpox required
  • Flu Vaccine: Required during flu season (10/1 - 3/31); declinations not accepted or must wear mask if declined (facility policy)
  • COVID-19 Vaccine: Must be fully vaccinated and boosted prior to start; exemptions possible with approval and facility-specific forms

Additional Requirements

  • Background checks including 7-year search, EPLS/GSA/SAM, HHS/OIG (annual)
  • Competency exams and medication/pharmacology tests as applicable
  • Professional references:
    • 2 for travel, allied, per diem acute care
    • 1 for per diem non-acute care
  • Skills checklist required annually
