Job Details
- Unit Type: Behavioral Health, Skilled Nursing Facility / Nursing Home
- Average Daily Census: 20
- Rooms/Beds: 20
- Staff Ratios:
- Days: 1:20
- Weekends: 1:20
- Weekend Coverage: Every other weekend
- Float Requirement: Yes, as needed by facility
- Local Candidates: Must live more than 75 miles away
- Electronic Health Record System: EPIC required
Experience & Requirements
- Years of Experience Required: 2 years in LVN / LPN specialty (highest requirement)
- Prior Travel Experience: Not required (unit accepts first-time travelers)
- Certifications Required:
- BLS (Basic Life Support) - AHA Provider/Instructor/RQI acceptable
- ACLS, PALS, NRP, TNCC, EFM, NIH Stroke Certification as required by unit description
- State License verification and NURSYS verification within 30 days of start
- Skills Required:
- Locked Unit experience
- Behavior management techniques
- Managing Assaultive Behavior
- Blood Glucose Monitoring/Testing
- De-Escalation Protocols
- Intake and Output monitoring
- Isolation Precautions
- Labeling of specimens
- Vital Sign monitoring
Health & Compliance Requirements
Candidates must complete health-related requirements and lab work within 5 days of signing the contract:
- Physical Exam (within 1 year, updated annually)
- Fit Mask testing (annual)
- Negative TB test or Chest X-Ray (within 1 year)
- TB Questionnaire (in lieu of annual TB test renewal)
- Immunizations or titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Drug Testing (within 30 days of start)
- Seasonal Flu Vaccination or signed declination (during flu season)
- Eye Exam (Snellen, within 1 year)
- COVID vaccination copy preferred but not required
Background & Documentation
- Government Issued Photo ID required
- 7-year Criminal Background Check within 3 months of start
- EPLS/GSA/SAM and HHS/OIG checks within 3 months of start, run monthly
- Education verification from primary source required
- Foreign equivalency evaluation if applicable
- Transcript/Diploma if no education verification on file
- Employment application and work history verification required
- References (2 within 3 years) required for Aya candidates only
- Skills checklist required annually
- Facility-specific documents including photo for badge and orientation safety manual acknowledgment
State-Specific Requirements (New York)
- NYS Nurse Aide Registry verification within 3 months of start for LTC and related units
- Fingerprinting may be required for certain units (Mental Health, Behavioral Health, Home Health Aide, LTC)
- Disciplinary Action Search and Sex Offender Registry Search within 30 days of start
- NY Medicaid Exclusion List check within 3 months of start, run monthly
