Job Details

• Shift: Days (5 days a week, 8-hour shifts)
• Hours: 8:00 AM - 4:30 PM or 9:00 AM - 5:30 PM
• Charting System: Epic (charting experience must be noted on profile)
• Weekend Requirement: Potentially 1 weekend per month
• Block Schedule: Accommodations available

• Time Off Restrictions:

  • No holiday time off
  • Maximum 5 total days off during the 13-week assignment
  • No time off requests on the day before, day of, or day after a holiday
  • No additional time off allowed after contract submission or while working

Required Experience & Qualifications

• Specialty: Therapy/Rehabilitation (COTA)
• Years of Experience: Minimum 2 years required
• Prior Travel Experience: Minimum of one successful travel assignment required; no first-time travelers
• Certifications:

  • BLS (Basic Life Support) required (AHA or American Red Cross accepted)
• Pending License: Accepted

Patient Population / Common Diagnoses

• CVA (Stroke)
• CABG and other thoracic cardiovascular surgeries
• Orthopedic surgeries
• Respiratory failure
• Chronic conditions such as COPD
• Nonhealing wound management
• Amputations

• Type: Acute Care Hospital
• Trauma Level: Level 3 Trauma Center
• Total Staffed Beds: 171
• Attendance Policy: Zero-fault attendance policy; any missed shift counts as unexcused. Two missed shifts result in a warning; three or more may result in cancellation.

Required Documentation & Compliance

• Background check within 30 days of start (7-year criminal search, multiple county checks, sex offender, nationwide, SS trace)
• Copy of diploma or transcript required (unofficial transcripts acceptable if graduation date is shown)
• Education verification if transcript/diploma unavailable
• Physical exam within 1 year stating "cleared to work" with physician signature
• Negative TB test or chest X-ray within 1 year (or chest X-ray within 5 years if prior positive)
• Required immunizations or titers: Measles, Mumps, Rubella, Hepatitis B, Varicella
• Drug test within 30 days of start (10-panel, negative results)
• Flu vaccination or approved exemption required during flu season (Oct 1 - May 1)
• Two professional references required prior to start
• Annual skills checklist and performance evaluation or references if applicable
• Work history verification for last 3 years (attestations acceptable, can be pending at start)
• Badge photo with specific requirements (plain white background, clear front-facing)
• Completion of facility-specific modules and trainings prior to start

• Charting experience in Epic must be clearly documented on the candidate profile
• No holiday time off allowed during the contract
