Job Opportunity: SLP Home Health (Travel/Contract)
Location: San Jose, California
Position Overview
- Specialty: Therapy/Rehabilitation (Speech-Language Pathologist - SLP)
- Employment Type: Travel/Contract
- Contract Length: 26 weeks (approximately 6 months)
- Coverage Areas: Multiple counties including Alameda, San Joaquin/Stanilaus, Santa Clara, Sonoma, and Marin Counties (territory discussed during interview; may include surrounding counties)
- Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
- Visits: Minimum 23-24 visits per week required to qualify for a 40-hour guarantee
- Average patients per day not emphasized; focus is on quality patient care
Experience & Requirements
- Specialty Experience: Minimum 2 years overall clinical experience with at least 1 year in Home Health
- Prior Travel Experience: Not required; first-time travelers are accepted
- Weekend Requirement: No weekends required
- On-Call: Not required (discretionary but not mandatory)
- Locals Accepted: Yes, within a 50-mile radius
- Licensure: Must have valid state license in hand at time of submission (no pending licenses accepted)
- Certifications Required:
- BLS (Basic Life Support)
- State License
- Dress Code: Any color scrubs, closed-toe shoes
- Charting System: Kinnser/Wellsky; all charting must be completed onsite (no remote charting allowed)
- Mileage Reimbursement: IRS standard from first patient to last patient (not to/from home)
- Parking: Free residential parking available
Compliance & Documentation
- All required documents must be completed and approved before starting patient care (compliance process may take 2-3 weeks)
- Required Documents Include (but not limited to):
- Government-issued photo ID (Driver’s License/State ID)
- Emergency Contact Form
- Background Checks (within 1 year, including SS Trace, County searches, Nationwide Sex Offender)
- Certifications: BLS, ACLS, PALS as applicable
- State License Verification
- Competency Exams as applicable
- Auto Insurance Card
- Physical Exam (within 6 months, signed by MD/DO, free of communicable diseases)
- Immunizations and Titers: Tetanus/Diphtheria/Tdap, TB test or Chest X-Ray, Measles, Mumps, Rubella, Hepatitis B, Varicella
- Drug Testing: 9 Panel Drug Screen within 1 year
- Seasonal Vaccinations: COVID vaccine or approved exemption, Flu vaccine or declination form during flu season (Oct 1 - Mar 31)

- Contract Length: 4-6 months per contract; less experienced clinicians must commit to 6 months (non-negotiable)
