Background Checks (within 1 year, including SS Trace, County searches, Nationwide, Sex Offender, EPLS/GSA/SAM, HHS/OIG)
Physical Exam (within 1 year)
Negative TB test or Chest X-Ray (within 1 year)
Immunizations or Titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
Drug Testing: 9 Panel Drug Screen within 1 year
Seasonal Flu Vaccination or Declination (during flu season)
Professional References:
- 2 for Travel, Allied, Per Diem Acute Care
- 1 for Per Diem Non-Acute Care
Skills Checklist: Required annually
Medication/Pharmacology Competency Exam (as applicable)

Additional Notes

Oasis experience preferred
EMR experience with HCHB required
Must adhere to dress code and technology policies
Flexibility and willingness to travel within the designated territory is essential
