Documentation & Compliance Requirements

  • Government issued photo ID (Driver’s License/State ID)
  • Emergency Contact Form (prior to start)
  • Background Checks (within 1 year, including SS Trace, County searches, Nationwide, Sex Offender, EPLS/GSA/SAM, HHS/OIG)
  • Physical Exam (within 1 year)
  • Negative TB test or Chest X-Ray (within 1 year)
  • Immunizations or Titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug Testing: 9 Panel Drug Screen within 1 year
  • Seasonal Flu Vaccination or Declination (during flu season)
  • Professional References:
    • 2 for Travel, Allied, Per Diem Acute Care
    • 1 for Per Diem Non-Acute Care
  • Skills Checklist: Required annually
  • Medication/Pharmacology Competency Exam (as applicable)

Additional Notes

  • Oasis experience preferred
  • EMR experience with HCHB required
  • Must adhere to dress code and technology policies
  • Flexibility and willingness to travel within the designated territory is essential
