Job Opportunity: Physical Therapist (Travel/Contract)

Job Summary

• Plan and conduct treatment programs to restore and improve physical, social, and mental functions.
• Serve as co-leader in interdisciplinary patient care groups.
• Provide therapy interventions using appropriate protocols and techniques.
• Establish individualized care and discharge plans and communicate them effectively.
• Complete comprehensive and accurate clinical documentation within deadlines.
• Supervise assigned therapy assistants.

Work Schedule

• Full-time, Monday to Friday, 8 AM to 5 PM
• Covering Willits/Ukiah service area
• Mileage reimbursement provided

Required Qualifications

• Education:

  • Bachelor’s Degree or equivalent (required)
  • Master’s Degree (preferred)
• Licenses/Certifications:

  • Physical Therapy licensure in California (required)
  • CPR or Basic Life Support (BLS) certification (required)
  • Valid Driver’s License, age 21 or older (required)
• Experience:

  • Must have Home Health experience

Essential Functions

• Conduct patient evaluations to ensure quality standards.
• Use clinical techniques and tools to develop individualized treatment plans.
• Provide therapy interventions and ensure communication of care plans.
• Supervise therapy assistants and perform other related duties as assigned.

Compliance and Requirements

• Must receive all required vaccinations as a condition of employment (medical and religious exemptions may apply).
• Background check covering the last 7 years, including social security trace, county/state/federal court jurisdictions, OIG, OFAC, SAM, FACIS 3, National Sex Offender, and DMV/MVR if driving is required.
• Drug testing required within 30 days before start.
• Physical exam required within 90 days of start.
• Fit mask testing (N95 preferred) required.
• Immunizations and titers required or declinations accepted for:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • TB test (QGold/TSpot or 2 PPDs within 30 days of start, updated annually)
  • Measles (Rubeola), Mumps, Rubella, Varicella (positive titer, vaccines, or declination)
  • Hepatitis B (positive titer, series, or declination)
• Seasonal flu vaccination or facility-specific declination/exemption required during flu season (Oct 31 - Apr 30).
• COVID-19 vaccination series required; medical or religious exemptions considered with facility-specific forms. Booster highly recommended.

Certifications (may be required depending on unit)

• ACLS (Advanced Cardiac Life Support)
• BLS (Basic Life Support)
• PALS (Pediatric Advanced Life Support)
• NRP (Neonatal Resuscitation Program)
• TNCC (Trauma Nursing Core Course)
• EFM (Electronic Fetal Monitoring)
• Management of Assaultive Behavior (MAB) or Crisis Prevention Institute (CPI)
• NIH Stroke Certification (annual update required)

Competencies

• Medication Test/Pharmacology Competency Exam (for RNs and LVNs)
• Color Blind Testing (all candidates)
• Unit-specific Competency Exams (e.g., Dysrhythmia/EKG, Conscious Sedation)
• Age Specific Competency Exam (update every 5 years)

Documentation and Compliance

• Photo for badge (passport style, high resolution, no filters/selfies)
• Confidentiality Agreement, Waiver of Benefits Acknowledgement, and Assignment of Intellectual Property Rights must be reviewed and signed prior to start.
• Complete application/resume with 7-year work history including locations and gaps.
• Two clinical references verified within one year of start.
• Unit-specific skills checklist required.
