Home Care Registered Nurse - Vermont
Job Type
- Contract position
- Contract length: 13 weeks
- Shift: 8am to 4pm, Sunday through Thursday, with some on-call required

Job Description
- Provide nursing care using the nursing process to assess patient and caregiver needs.
- Develop and communicate realistic, evidence-based plans of care including patient/caregiver involvement.
- Serve as Case Manager for assigned patients, managing caseloads and visit times per productivity standards.
- Complete OASIS documentation accurately and timely, staying current with documentation requirements.
- Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective quality care.
- Maintain and update skills in IV use and maintenance, wound VAC, complex wound care, and infection control.
- Collaborate with LPNs within their scope of practice, delegate tasks, and review documentation.
- Evaluate and adjust plans of care based on patient responses and identified problems.
- Maintain accurate nursing care documentation for effective communication within the healthcare team.
- Follow agency protocols for visit documentation and completion.
- Educate patients and caregivers on medication side effects, treatment options, discharge instructions, and reportable conditions.
- Identify and communicate interdisciplinary patient needs (e.g., physical therapy, social work) promptly.
- Complete required agency, state, and federal forms (e.g., HHABN, Medicare Notice of Non-Coverage) timely.
- Manage visits efficiently, ensuring supervisory visits are not combined with routine visits.
- Assess patient needs for Home Care Aide services, develop and coordinate plans of care, and supervise accordingly.
- Report patient incidents (infections, injuries) promptly to Quality Improvement specialists.
- Coordinate safe patient discharges and transfers, ensuring all paperwork and information are processed correctly.
- Collaborate with physicians, providers, team members, and vendors to support the plan of care.
- Participate in monthly peer review audits and case conferences to improve communication and care quality.
- Advocate proactively for patient care issues sensitive to individual patient and family needs.
- Participate actively in interdisciplinary team (IDT) meetings to develop and update individualized plans of care.
- Stay current on pain control, symptom management, and high-tech nursing skills relevant to home care.
Required Qualifications
- Valid Registered Nurse (RN) license in Vermont
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum of 2 years of Home Health nursing experience
- Home Health nursing experience within the last year
- Experience with IV use and maintenance
- Experience with OASIS documentation and processes
