Job Opportunity: Primary RN - Case Manager

Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks


Job Description

As a Primary RN Case Manager in hospice care, you will provide compassionate, patient-centered nursing that supports patients and families during end-of-life care. This role involves:

  • Delivering expert symptom management and holistic care across a variety of settings including private homes, nursing homes, group homes, and assisted living facilities.
  • Acting as a clinical generalist with strong assessment and critical thinking skills.
  • Teaching and supporting patients and caregivers to identify personal goals of care and build confidence in caregiving.
  • Leading an interdisciplinary team for your assigned caseload of patients.
  • Working primarily Monday through Friday, 8am-5pm, with occasional weekend backup and on-call duties.

Required Qualifications

  • Valid RN license in Colorado
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Minimum 2 years of case management experience, including experience within the last year
  • Hospice experience
  • Strong clinical skills in symptom management and patient assessment
  • Ability to work independently and lead a care team
  • Compassionate and patient-focused approach
