<h3>Job Description – Registered Nurse (Home Health) | Smyrna Health &amp; Wellness Center, Smyrna, DE</h3><h3>Assignment Details</h3><ul>	<li>Facility: Smyrna Health &amp; Wellness Center – Smyrna, Delaware</li>	<li>Position: Registered Nurse – Home Health</li>	<li>Duration: 13 Weeks</li>	<li>Start Date: ASAP (Scheduled Orientation)</li>	<li>Schedule: Monday–Friday, 8-hour day shifts</li>	<li>Guaranteed Weekly Hours: 40</li>	<li>Pay Cycle: Weekly Pay</li>	<li>Coverage Area: Lower New Castle County, Kent County, and up to a 60-mile radius from Smyrna (including Southern Delaware).</li>	<li>Required Experience: Recent Home Health RN experience with OASIS proficiency.</li>	<li>License Required: Active Delaware RN License.</li></ul><h3>On-Call &amp; Weekend Requirements</h3><ul>	<li>Participate in on-call rotation approximately 2 times per month (maximum of 3).</li>	<li>Initial after-hours calls are screened by a non-clinical call center, with RNs receiving only true clinical concerns.</li>	<li>On-call hours are generally 4:30 PM – 8:00 AM (occasionally split into evening and overnight shifts).</li>	<li>No patient home visits are conducted between 8:30 PM and 8:00 AM; overnight responsibilities are limited to telephone triage.</li>	<li>Most overnight on-call shifts involve 1–2 patient calls during the early evening, with very few calls overnight.</li>	<li>Participate in one weekend rotation every four weeks, typically Saturday and Sunday, with the option of working two Saturdays every four weeks based on scheduling needs.</li>	<li>An alternate weekday off is generally provided to ensure nurses do not work more than five consecutive days.</li></ul><h3>Credentialing Requirements</h3><ul>	<li>Allow approximately 5 weeks for credentialing.</li>	<li>Non-Delaware residents must travel to Delaware after acceptance for in-person fingerprinting.</li>	<li>Fingerprint processing typically requires 3–4 weeks before credentialing is complete.</li></ul><ul>	<li>Provide skilled nursing care to adult and geriatric patients in a home health setting throughout Lower New Castle County, Kent County, and surrounding areas within a 60-mile radius of Smyrna, DE.</li>	<li>Perform comprehensive patient assessments, develop individualized plans of care, and implement physician-ordered treatments.</li>	<li>Complete OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and Follow-up assessments accurately and in compliance with Medicare guidelines.</li>	<li>Administer medications, wound care, IV therapy, injections, catheter care, ostomy care, and other skilled nursing interventions in the patient's home.</li>	<li>Monitor patient conditions, identify changes in health status, and communicate significant findings promptly to physicians and the interdisciplinary care team.</li>	<li>Educate patients and caregivers on disease management, medication administration, chronic illness management, home safety, infection prevention, and self-care techniques.</li>	<li>Coordinate patient care with physicians, physical therapists, occupational therapists, speech therapists, social workers, and case managers to achieve optimal outcomes.</li>	<li>Develop, update, and evaluate individualized care plans based on patient progress and clinical needs.</li>	<li>Maintain accurate, timely, and compliant clinical documentation while meeting productivity expectations within an 8-hour workday.</li>	<li>Ensure compliance with Medicare Conditions of Participation, agency policies, HIPAA regulations, and infection prevention standards.</li>	<li>Travel independently between patient homes while managing an assigned caseload efficiently within the designated service area.</li>	<li>Participate in interdisciplinary case conferences, quality improvement initiatives, and discharge planning activities.</li>	<li>Deliver compassionate, patient-centered care while promoting independence, reducing hospital readmissions, and improving overall quality of life.</li></ul> #TB_RN
