Job Opportunity: Registered Nurse - Home Health (Travel)

Job Details

• Start Date: ASAP
• Guaranteed Hours: 38 hours per week, flexible up to 40 hours
• Shift: Variable shifts, 5-8 hours, Monday through Friday
• Patient Load: 20-25 patients, averaging 4.5 visits per day
• Travel: Daily travel between 25-60 miles depending on territory and patient census
• Float Requirements: May float to other territories when patient census is low
• On Call/Call Back:

  • 1-2 times during the week and every other weekend
  • Weekend clinician on call for admissions/routines on assigned weekend
  • Response time: 15 minutes to return calls
  • Calls mostly triage; may require phone consultation or patient visit
• Weekend/Holiday: Every other weekend on call and holiday rotation

Requirements

• Experience: Minimum 2+ years in Home Health (preferred)
• Prior Travel Experience: Required
• Certifications:

  • BLS
  • Active Wisconsin or Compact RN license
  • Valid Driver’s License
  • OASIS certification is a plus
• Charting Experience: OASIS required; Epic preferred
• Skills:

  • OASIS admissions, episode management, documentation in EMR
  • Clinical competencies in wound care, IVs, ports, lab draws, chronic disease management
  • Coordination of care and physician communication
• Personal Attributes: Dependable, accountable, compassionate, resilient, adaptable to change, positive attitude, social

Additional Information

• Must live outside a 50-mile radius from Wisconsin Rapids
• Flu and COVID vaccination required during flu season or mask must be worn at all times
• Orientation week hours not guaranteed
• Eligible for 36 hours of unbillable low census hours during contract
• Call back pay applies with a minimum of 2 hours if called back during low census hours
