Licensed Practical Nurse (LPN) - Per Diem

Location: Wabasha, Minnesota

• Employment Type: Per Diem
• Contract Length: 6 weeks (45 days)

• Shift Details: Per Diem shifts
• Float Requirements: Required to float to other units as needed
• On Call/Call Back: None
• Weekend/Holiday Requirements: Per Diem

Unit Overview

• 200 Wing: Mixed occupancy including residents requiring total assistance, short-term rehab, and end-of-life care

  • Staffing ratio: 1 CNA to 8 residents
• 100 Wing: Residents mostly ambulatory and cognitively intact, not total care

  • Staffing ratio: 1 CNA to 12 residents

Requirements

• Valid LPN / LVN license in Minnesota
• LPN / LVN with a minimum of 2 years of experience, including experience within the last year
• Associate Degree or Certificate in Nursing
• BLS certification issued by the American Heart Association
• Experience in Long Term Acute Care (LTAC) settings
• Experience in Post Acute Care settings
• Experience in Long Term Care (LTC)
• Skilled Nursing Facility (SNF) experience

• Scrub colors: Any professional color
• Orientation week is not guaranteed
• Set hours per week guaranteed except during holidays when the unit is closed
• Medical compliance must be started within 48 hours of offer acceptance (24 hours for quick start)
