Job Opportunity: IMC - RN - Case Manager Nurse

Location: Brooklyn, New York

Job Summary

As a Case Manager Nurse, you will advocate for safe, timely adherence to patient care plans, performing face-to-face patient assessments and collaborating with interdisciplinary teams. Responsibilities include reviewing electronic health records (EHR) for medical necessity, coordinating with social workers, physicians, and insurance companies, and facilitating discharge planning and community resource referrals.

Key Responsibilities

• Conduct face-to-face patient visits to assess physical, mental, and emotional barriers to self-care.
• Participate in interdisciplinary rounds and daily EHR reviews for medical necessity (severity of illness and intensity of service).
• Collaborate with social workers to identify social determinants of health affecting patient outcomes.
• Serve as a liaison between physicians and physician advisors.
• Monitor and document clinical information in EHR for insurance submissions (initial, concurrent, retroactive reviews).
• Review admissions for medical necessity within 48 hours.
• Apply knowledge of Milliman Care Guidelines and CMS guidelines for post-acute placements.
• Communicate admission guidelines and provide real-time interventions to prevent delays and ensure compliance.
• Assist in discharge planning, including arranging community services and referrals to home care and infusion companies.
• Document case management notes concisely and maintain communication with nursing staff regarding discharge plans.
• Advocate for patients and families with healthcare disciplines and community agencies.
• Mandated reporter of suspected abuse (domestic violence, elder, child abuse, etc.).

Contract Length

• 13 weeks

Qualifications & Experience

• Bachelor’s degree in Nursing required.
• Minimum 3 years of experience in one or more of the following areas:

  • Acute Care
  • Medical-Surgical
  • Intensive Care
  • Emergency Department
  • Community Nursing (Home Care)
• PRI certification required.

Licenses & Certifications

• Current New York State RN License required.
• Basic Life Support (BLS) required.
• Advanced Cardiovascular Life Support (ACLS) required.
• Pediatric Advanced Life Support (PALS) preferred.
• Case Manager Certification (CCM) preferred.
