Job Description

• Provide direct clinical social work services, counseling, discharge planning, and case management to hospital patients and their families.
• Assess and document emotional, social, financial, environmental, and follow-up referral needs.
• Collaborate with the interdisciplinary healthcare team to formulate and implement psychosocial plans of care.
• Develop and monitor discharge plans and progress toward goals of care.
• Facilitate integration with community-based resources to optimize patient health.
• Support patients and families through a holistic approach addressing mind, body, and spirit.
• Participate in quality improvement, clinical research, and community networking activities.
• Provide psychological assessment and support for patients across acute, long-term, and community health settings.
• Complete required documentation in patient medical records.
• Demonstrate ongoing professional skill enhancement and comply with department policies.

Required Qualifications

• Valid Social Worker license in California.
• Minimum of 1 year of social work experience, including experience within the last year.
