Job Opportunity: PT Home Health

Schedule

• Day shift, 5 days per week (8:00 AM – 5:00 PM)
• 40 hours per week guaranteed
• Occasional weekends may be required

Patient Load & Case Types

• Average 5-7 patients per day (to be confirmed during interview)
• Geriatric cases

Territory

• Up to a 60-mile radius from the office hub (typically about 30 miles)
• Flexibility with territory coverage expected

Required Qualifications

• Valid Physical Therapist license in Montana
• Minimum 1 year of Physical Therapist experience within the last year
• BLS (Basic Life Support) certification
• Experience with Homecare Homebase (HCHB)
• Experience with OASIS

Equipment Provided

• iPad provided for work use
• Cell phone use not permitted for work

Background and Compliance Requirements

• Comprehensive background checks required within 90 days of start, including:

  • SSN trace, county searches (7 years), FACIS Level 1, nationwide sex offender, motor vehicle records, education and work history verification
  • State-specific searches as applicable
  • EPLS/GSA/SAM and HHS/OIG checks
• Fingerprinting may be required depending on state placement
• Education verification of highest degree related to profession
• Signed acknowledgment of Orientation Safety Manual (updated annually)
• Medication and competency exams as applicable
• Agency background attestation required prior to start

Health and Immunization Requirements

• Physical exam within last 12 months (physician statement)
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire
• Proof of immunity or vaccination (or declination) for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within last 12 months
• Flu seasonal vaccination or facility-specific declination form during flu season
• COVID vaccination or facility-specific declination

References and Work History

• Two professional references required (within 2 years of start)
• Full 7 years of work history with no gaps, including city and state details
• Work history verification within 90 days of start

This position welcomes first-time travel therapists and local candidates at travel rates. Mileage reimbursement is provided at the IRS standard rate.
