Physical Therapist Assistant (PTA) - Skilled Nursing Facility (SNF)

Location: Los Angeles, California
Employment Type: Travel/Contract
Assignment Length: 13 to 26 weeks
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Start Date: ASAP (flexible for later starts)
Schedule Notes: May include occasional weekend coverage and floating

Job Description

Provide therapy/rehabilitation services in a skilled nursing facility setting.
Work primarily day shifts with a guaranteed 36 hours per week.

Required Qualifications

Valid Physical Therapy Assistant license in California

Compliance and Documentation Requirements

Government-issued photo ID (Driver’s License or State ID).
Emergency contact form completed prior to start.
Background checks including 7-year work/living/school history, sex offender registry, and annual EPLS/GSA/SAM and HHS/OIG checks.
Physical exam within 1 year of start date.
Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if prior positive, negative chest x-ray within 1 year required.
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
9-panel drug screen within 1 year of start.
Seasonal flu vaccination or declination during flu season (October 1 - March 31).
Completion of facility-specific compliance quiz related to wage and hour laws.
Annual skills checklist and competency exams as applicable, including medication/pharmacology competency if required by profession.

References

2 professional references required for travel, allied, or per diem acute care roles.
1 professional reference required for per diem non-acute care roles.
References should be within 3 years of start date.
