Physical Therapist - Home Health (California, San Jose Area)

Job Type

  • Travel/Contract position
  • Contract length: 4-6 months (less experienced clinicians must commit to 6 months)

Work Schedule

  • Day shift: 8:00 AM to 5:00 PM (5 days a week, 8 hours per day)
  • No weekend work required
  • Minimum 23-24 patient visits per week to qualify for a 40-hour guarantee

Patient Population and Caseload

  • Home Health setting
  • Types of cases: Wound care, geriatric home health patients
  • Focus on patients per week rather than per day

Location and Travel

  • Position based in the Sacramento area with possible travel to surrounding counties
  • Locals within a 50-mile radius are accepted
  • Mileage reimbursed from first patient to last patient (not to/from home)

Required Qualifications

  • Valid Physical Therapist license in California
  • Minimum 1 year of Physical Therapist experience within the last year
  • Experience in a Home Health setting
  • BLS certification

Clinical and Professional Expectations

  • Patient care focused
  • Punctual and reliable
  • Self-starter with strong follow-through on charting
  • Charting must be completed onsite using provided tablet (no remote/charting from home)

Dress Code and Work Environment

  • Any color scrubs allowed
  • Closed-toe shoes required
  • Free residential parking available

Compliance and Health Requirements

  • Background checks including SS trace, county searches, nationwide sex offender registry, and annual federal exclusions screening
  • Physical exam within 6 months signed by MD/DO confirming freedom from active communicable diseases
  • Immunizations/vaccinations required or accepted declinations for:
    • COVID-19 (medical/religious exemptions allowed)
    • Influenza (declination form required if opting out)
    • Tetanus/Diphtheria/Tdap (within 10 years or exemption)
    • Measles, Mumps, Rubella (proof of immunity or vaccination per birth year guidelines)
    • Hepatitis B (titer, vaccine series, or declination)
    • Varicella (titer, vaccination, history of chickenpox, or declination)
  • Negative TB test or chest x-ray within 1 year of start (with specific rules for past positives)
  • 9-panel drug screen within 1 year of start (instant drug screen acceptable)
  • Annual skills checklist and competency exams as applicable

Documentation Required Before Start

  • Government-issued photo ID (Driver’s License/State ID)
  • Emergency contact form
  • Auto insurance card
  • Background checks and federal screenings
  • All required certifications and licenses verified through primary source
  • Compliance documents including immunizations, physical exam, drug screen, and others as listed above

Additional Notes

  • On-call duties are not required (discretionary)
  • Return-to-Office (RTO) requests must be submitted at time of application; no additions accepted after offer
  • Patient care and timely charting are highly valued qualities in clinicians for this role
