Overview
- Travel/contract role in San Leandro, California
- Day shift: 5x8 hours, 8:30 AM to 5:00 PM
- 40 hours per week
- Contract length: 13 weeks
- First-time travelers accepted
Required Qualifications
- Valid RN license in California
- Associate of Science in Nursing / Associate Degree in Nursing
- Case Management (1 year of experience)
- Case Management experience within the last year
- Experience in an Acute Care setting
- BLS issued by American Heart Association
- Experience with Epic
Responsibilities
- Acute care case management
- Admission criteria review
- Discharge planning
- Disease management
- Needs assessment
- Plan of care coordination
Preferred Experience
- Emergency room
- Behavioral health
- Workers’ compensation case management
Additional Requirements
- Must be able to complete patient-facing fit testing requirements
- Must meet required health screening, immunization, drug screening, background check, education verification, and onboarding documentation requirements before start
