Job Details

  • Specialty: Registered Nurse (Home Health)
  • Shift: Friday/Saturday/Sunday or Saturday/Sunday/Monday
  • Hours: Approximately 12-hour shifts, 40 hours guaranteed per week
  • Visits: 2 Start of Care (SOC) visits per day plus an additional 2 visits on average
  • Patient Population: Geriatric cases
  • Territory: Up to a 60-mile radius from the office/hub
  • Dress Code: Scrubs (any color), no jeans
  • Technology Provided: iPad provided for charting; cell phones not allowed
  • Charting Deadline: All charting must be completed by 9 AM the following morning
  • Communication: Timely communication is required
  • Mileage: Reimbursed at IRS standard rate
  • Locals: Local nurses accepted at travel rate
  • On-call: None
  • Holidays: No holiday shifts required
  • Flexibility: Travelers must be flexible with territory coverage

Required Qualifications

  • Valid Registered Nurse (RN) license in Wisconsin
  • Minimum 1 year of recent experience in Home Health nursing within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
  • Experience with Homecare Homebase (HCHB) electronic medical record system

Compliance and Background Requirements

  • Background checks must be completed within 90 days of start and updated annually, including:
    • SSN Trace
    • County searches for last 7 years of residence/work/school
    • FACIS Level 1
    • Nationwide Sex Offender Registry
    • Motor Vehicle Records (MVR)
    • Education and Work History Verification
    • State-specific searches as applicable (e.g., Kentucky, Virginia, Indiana, New Hampshire, South Carolina)
  • Fingerprinting may be required depending on state placement, with specific instructions for some states
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
  • Education verification of highest degree related to profession required
  • Employment history verification for 7 years required with no gaps
  • Professional references required (2 for travel/allied/per diem acute care; 1 for per diem non-acute care) within 2 years of start

Health and Safety Requirements

  • Physical exam within the last 12 months (not required annually)
  • Negative TB test or chest X-ray within 1 year of start (chest X-ray within 5 years if positive PPD)
  • Annual TB questionnaire required
  • Immunizations or proof of immunity required for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • 9-panel drug screen within 12 months of start (instant acceptable)
  • Flu seasonal vaccination or facility-specific declination form required during flu season
  • COVID vaccination or facility-specific declination required

Additional Requirements

  • Proof of current auto insurance required due to driving responsibilities
  • Signed acknowledgment of Orientation Safety Manual (updated annually)
  • Completion of facility-specific Acceptable Use Policy prior to start
  • Facility-specific forms may be required depending on placement state
