Job Opportunity: Speech-Language Pathologist - Home Health
Location: San Jose, California (with possible travel to surrounding counties)
Employment Type: Travel/Contract
Contract Length: 4 to 6 months (less experienced clinicians must commit to 6 months)
Job Description
- Provide home health therapy services primarily in Speech-Language Pathology.
- Patient caseload focuses on home health, wound care, and geriatric patients.
- Minimum of 23-24 patient visits per week required to qualify for a 40-hour guarantee.
- No weekend work required.
- On-call duties are discretionary and not required.
- Locals within a 50-mile radius are accepted.
- Charting must be completed onsite using provided technology; no remote charting allowed.
- Dress code: Any color scrubs and closed-toe shoes.
- Free residential parking available.
- COVID-19 vaccination required; religious exemptions allowed. Flu vaccination is optional with declination.
Required Qualifications
- Valid Speech Language Pathologist license in California.
- Minimum 1 year of Speech Language Pathologist experience within the last year.
- Experience in a Home Health setting.
- Basic Life Support (BLS) certification required.
- Physical Therapist experience.
Compliance & Documentation Requirements
- Government-issued photo ID.
- Background checks including SS trace, county searches, and national registries within 1 year.
- Annual verification of exclusion lists (EPLS/GSA/SAM, HHS/OIG).
- Certifications: BLS required; ACLS and PALS preferred or as applicable.
- Physical exam within 6 months prior to start, including clearance of communicable diseases.
- Immunizations or proof of immunity required for:
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Measles, Mumps, Rubella (proof of immunity or vaccination)
- Hepatitis B (titer, vaccination series, or declination)
- Varicella (titer, vaccination, history, or declination)
- Negative TB test or chest X-ray within 1 year.
- Drug screening (9-panel) within 1 year.
- Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31).
- COVID-19 vaccination or approved medical/religious exemption.
- Auto insurance card required.
- Emergency contact form required prior to start.
- Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care.
Additional Notes
- Mileage reimbursed for travel between patient visits (not to/from home).
- All required documents must be submitted before starting patient care.
- Requests for time off (RTO) must be submitted with application; no additional RTO accepted after offer.
- Emphasis on clinicians who are patient-focused, punctual, self-starters, and diligent with charting.
