Job Opportunity: Registered Nurse - Home Health

Location: Hicksville, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 3 months)

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the home clinic; expected weekly travel is approximately 150-200 miles.
• Schedule is typically Monday through Friday, 8 AM to 5 PM, with weekend and on-call requirements varying by location and discussed during the interview.
• Use of electronic medical records system HomeCare HomeBase (HCHB) required; proficiency in OASIS is required for RNs.
• Dress code: Scrubs.

Required Qualifications

• License: Current Registered Nurse (RN) license valid in New York. Pending licenses are not accepted.
• Experience: Minimum 1 full year of recent Adult Home Health experience or 1 full year of Hospice experience, depending on specialty.
• First-time travelers are not accepted; must have recent specialty experience.
• Certifications: Current CPR and BLS certification required.
• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; if not current, retraining is required.
• Must be able to meet documentation and compliance requirements.

Additional Requirements

• Must complete a variety of background checks, including but not limited to:

  • 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
  • Fingerprinting as required by state regulations.
  • Drug screening (10-panel).
  • Education verification.
• Health documents required prior to start:

  • Physical exam within 12 months.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination.
  • COVID-19 vaccination series or declination.
• Must have personal or auto insurance policy valid and current.
• Must review and sign various compliance, confidentiality, and policy documents prior to start.
• Two professional references required.
• Work history verification for at least the last 7 years or two most recent employers.

Notes

• Weekend and on-call requirements vary and will be discussed during the interview.
• Local candidates are accepted under the same terms as travelers.
• No guaranteed hours; if patient census is low, floating to nearby clinics may be arranged.
• Candidates must be willing to comply with all state-specific and agency-required documentation and training.
