Job Details
- Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week (40 hours/week)
- Patient Ratio: 1:1, with up to 6 patient visits per day (30 productivity points standard)
- Schedule: No weekend requirement, no on-call, no floating between units, no block scheduling
- Local Candidates: Accepted within a 50-75 mile radius
- Start Date: ASAP
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Must have own transportation; daily travel radius up to approximately 60 miles (60-90 minutes total drive time)
- Attire: Business casual; scrubs allowed but no specific color required; no jeans
- Charting System: EPIC Remote Client (laptop), Rover app (phone), and Oasis documentation required
Required Qualifications
- Valid Registered Nurse (RN) license in Oregon
- Minimum 1 year of Home Health nursing experience, with experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Basic Life Support (BLS) certification issued by the American Heart Association or Canadian Heart and Stroke Foundation
- Experience in a Home Health setting
- Experience with OASIS documentation
