Health and Immunization Requirements

Medical fit mask test within 1 year prior to start (N95 or PAPR required)
Tetanus/Diphtheria/Tdap vaccine or declination
Negative TB test (Q-gold/T-spot within 30 days of start or two PPDs including one within 30 days)
TB questionnaire updated annually
Measles (Rubeola), Mumps, Rubella, Varicella titers required; boosters if low titers
Hepatitis B titer, vaccine series, or declination accepted
Seasonal flu vaccine required (declinations not accepted except approved religious/medical exemptions)
Drug testing within 30-60 days of start (9-panel minimum; instant drug screens not accepted)
Latex questionnaire required prior to start

Competency and Orientation

Medication/Pharmacology competency exam required for RNs
Color blindness testing required
Abuse reporting competency required (covering physical/sexual abuse, domestic violence, neglect/exploitation)
Facility-specific exams at orientation including:
- Pharmacology exam
- Point of Care Testing
- Airborne Precaution Mask Fitting
- Basic Knowledge Assessment Tool (BKAT) for ICU, Tele/Step-Down, Progressive Care units
Orientation includes completion of assigned learning modules (~6 hours)
