Job Opportunity: Pediatric Outpatient Physical Therapist (Travel/Contract)

Schedule:
- Monday to Friday
- 5 shifts per week, 8 hours per shift
- Shift hours: 9:30 AM to 6:00 PM
- No on-call required
- No floating required
- Local candidates accepted within a 50-mile radius
Experience Requirements:
- Minimum 1 year of experience required
- First-time travelers accepted
Licensure & Certifications Required:
- California Physical Therapist License (CA PT License)
- BLS (Basic Life Support) certification required
- Pediatric and infant therapy experience mandatory
Additional Certifications (as applicable):
- ACLS (Advanced Cardiac Life Support)
- PALS (Pediatric Advanced Life Support)
- NRP (Neonatal Resuscitation Program)
- Other certifications may be required based on clinical track and facility needs
Documentation & Compliance Requirements:
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Education verification required
- Employment application covering at least last 5 years
- Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health screenings including:
- TB test or chest X-ray (updated annually)
- TDAP vaccine (within 10 years) or declination
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine or declination (required during flu season)
- COVID-19 vaccine or declination (required if onsite)
- Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
- Fit mask test (respirator clearance) required for travel candidates starting on or after 2/5/2024
- Competency exams and medication/pharmacology tests as applicable
- Providence-specific WorkRamp training modules must be completed prior to start (includes core, specialty, and regulatory modules depending on role and location)
- Epic charting system training required (transcripts must be submitted and validated prior to moving to paid status)
- Photo for badge (passport style with neutral background) may be required
Work Environment:
- Charting system: Epic
- Scrub colors: Navy blue for RNs; others provided as needed
- Free parking available
Additional Notes:
- Facility allows calling off 1 shift per 2 weeks (guaranteed hours with some flexibility)
- All requested time off (RTO) must be submitted at time of application and approved based on facility needs
- Floating policy acknowledgment required; may be asked to float to other Providence facilities within reasonable distance
