- BLS (Basic Life Support) - AHA only, RQI acceptable
- Trauma Level I Experience required (at least 1 year in Level 1 trauma, screening > 2 years total experience)
- Circulate role required; Scrub preferred
- License: Must have an active Indiana RN license before submission (no pending licenses accepted)
- Call Requirements:
- Call shifts required (6 call shifts in a 6-week period, can vary)
- Must work 2 out of 3 major holidays
- Scheduling: No block scheduling
- Locals Accepted: Yes (travelers residing in Indiana may have adjusted pay)
- Floating: Yes, floating within scope to meet facility needs (except CV & Transplant areas)
- First Timers: Accepted

- General Surgery, Plastic Surgery, Gynecology, ENT, Trauma, Neurosurgery, Orthopedics, GU, Colorectal Robotics
- Average Daily Census: 70-80 patients

Important Notes
- Perioperative/OR RN travelers are exempt from the Indiana RN restriction effective 11/30/2024 and may continue working past this date.
- Must be well-rounded and current in most OR areas except cardiovascular and transplant, as floating may be required.
Required Documentation & Compliance
- Background Checks:
- Must include 7 years of residence/work history
- Social Security Number trace, county criminal, statewide, national sex offender, FACIS Level III
- Education verification (new travelers only)
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- Health Documents:
- Physical exam within 12 months of start
- Fit mask test within 12 months (specific approved respirators required)
- Negative TB test or chest X-ray within 1 year
- Immunizations/vaccinations required:
- Rubeola (Measles), Mumps, Rubella: Positive titer or 2 vaccines required (no declinations)
- Varicella: Positive titer or 2 vaccines required (no declinations)
- Hepatitis B series, titer, or signed declination required
- Tetanus/Diphtheria/Tdap preferred but not required
- Seasonal Flu vaccination/documentation required (no declinations accepted)
- Negative 10-panel drug screen within 12 months
- Certifications:
- ACLS, PALS, NRP, CHEMO, TNCC, NIH Stroke, EFM as applicable (verify per cost center)
- Competencies:
- Medication/Pharmacology test required
- Unit competency exam required
- Color blind testing (Ishihara or equivalent) required
- Facility-Specific Requirements:
- Completion of orientation packets, compliance checklists, confidentiality agreements, and other facility forms prior to start
- Mandatory completion of 22 RRT modules for Respiratory Therapists (if applicable)
- References: Minimum 2 references from recent employment with supervisory titles, verified within 2 years
- Employment Application: Must be current and explain any gaps over 30 days within the last 7 years
- Skills Checklist: Required within 12 months of start and annually thereafter
