Required Qualifications
- Valid RN license in Oklahoma.
- Telemetry experience with 2 years of experience.
- Telemetry experience within the last year.
- BLS issued by American Heart Association.
- NIH Certification.
- Associate of Science in Nursing / Associate Degree in Nursing.
- Driver's License.
Onboarding / Compliance
- Ascension Pharmacology Exam: prior to start, minimum 80%, 2 attempts allowed, updated every 3 years.
- Ascension Specialty Exam: prior to first assignment, minimum 80% or national benchmark passing score, 2 attempts allowed, updated every 3 years.
- Ascension Color Vision: prior to start, updated every 3 years.
- Ascension Core Competencies: within a year of start, minimum 80% or national benchmark passing score, updated every 3 years.
- NIH Certification: required for onboarding.
- Ascension EKG Exam: required for onboarding, minimum 80%, 2 attempts allowed, updated every 3 years.
- Ascension Onboarding Requirements Education Attestation.
- Ascension Nursing Contract Worker Onboarding.
- Ascension Nova StatStrip: required annually.
- Ascension Cerner Test-Out.
- Ascension Badge Photo.
- COVID-19 Vaccine: fully vaccinated, no declinations accepted.
- MMR: two vaccines or positive IGG titer.
- Fit Test: required for all travelers, updated annually.
- Hepatitis B: vaccination series completion or signed declination.
- Influenza Vaccine: declinations/exemptions accepted; mask required at all times.
- Health Statement: statement of good health or completed physical exam, updated annually.
- Driver's License/State ID: must not be expired and must match address on file.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): TB questionnaire updated annually; if positive TB record, chest X-ray within last year required.
- Varicella: documented immunity, vaccination, or provider verification of history.
- Tdap: valid for 10 years.
- Criminal background check results.
- EPLS.
- OFAC.
- Education verification within 90 days of start.
- Confirmation of assignment on agency letterhead.
- Employment verification for the past seven years.
- 12 panel drug screen within 30 days prior to start, updated annually.
Optional / Preferred
- Hiring Manager Evaluation: post-assignment evaluation.
- Unit Specific Checklist: optional for onboarding.
- Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire: optional.
- Name change document: legal documentation of name change.
