Location: Jacksonville, Florida
Contract Length: 13 weeks


Job Details

Required Qualifications

  • License: Current RN license in Florida with primary source verification within 30 days prior to start
  • Certifications:
    • BLS (American Heart Association)
    • ACLS (American Heart Association)

  • Ascension Nova StatStrip training
  • Ascension Cerner Test-Out (clinical documentation system)
  • Health and Immunization Requirements:
    • MMR: Proof of two vaccines or positive titer, no declinations accepted
    • Fit Test: Annual respiratory fit test required
    • Hepatitis B: Vaccination series or signed declination
    • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31), exemptions require mask wearing
    • TB Screening: Positive IGRA or PPD results, negative chest X-ray within 1 year, annual TB questionnaire
    • Varicella: Proof of immunity or vaccination, no declinations accepted
    • Tdap: Valid within 10 years, no declinations accepted
  • Drug Screening: 12 panel drug screen within 30 days prior to start, updated annually
  • Background Checks and Verifications:
    • Criminal background check including SSN trace, OIG search, National Sex Offender Registry (updated annually)
    • EPLS (Excluded Parties List System) check within 90 days prior to start
    • Employment and education verifications within 90 days prior to start
  • Other Requirements:
    • Driver’s License/State ID (valid, must match address requirements)
    • Health Statement or physical exam prior to first assignment, updated annually
    • Ascension Skills Checklist (clinical skills verification) within 1 year of start, updated annually
    • Ascension Profile (resume or professional profile) submission required
