Required Qualifications
- License: Current RN license for Indiana with primary source verification within 30 days prior to assignment start
- Certifications:
- BLS (American Heart Association)

Health and Immunization Requirements

- Hepatitis B vaccination series or signed declination
- Influenza vaccine (seasonal; exemptions accepted with mask requirement)
- Positive IGRA or PPD results with negative chest X-ray within 1 year, plus annual TB questionnaire
- Two doses of Varicella vaccine or serological evidence of immunity (no declinations accepted)
- Tdap vaccine valid within 10 years (no declinations accepted)
- Health statement or physical exam prior to first assignment, updated annually
Background and Verification Requirements
- Nursys license verification within 30 days prior to start
- License/Certificate primary source verification within 30 days prior to start
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and others within 30 days prior to start; updated annually or if break in service > 90 days
- EPLS check within 90 days prior to start; rerun if break in service > 90 days
- Education verification within 90 days prior to start; rerun if break in service > 90 days
- Employment verification for past 7 years; updated annually or if break in service > 90 days
- Resume/Profile submission required
- At least two professional references prior to first assignment
- Driver’s License/State ID matching home state for compact license (if applicable)
Drug Screening
- 12 Panel Drug Screen within 30 days prior to start; includes amphetamines, cocaine, marijuana, PCP, methamphetamine, barbiturates, benzodiazepines, buprenorphine, methadone, oxycodone, morphine, and ecstasy
- Must include MRO review for prescriptions or negative-dilute results
