Radiology Technician - Travel Position

Job Description

• Staffing CT and X-ray for night shifts on Thursday and Friday

Required Certifications and Credentials

• BLS (Basic Life Support): Must be AHA certified
• Current License and Primary Source Verification: Within 30 days prior to assignment start
• Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly (minimum 80% score, max 2 attempts)
• Ascension Medical Imaging Contract Worker Onboarding Education Attestation: 6 hours pre-hire education with signed attestation
• Driver's License/State ID: Must be valid, not expired, and address must match licensing requirements

Health and Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Positive IGRA or PPD results, chest X-ray within a year, TB questionnaire completed; updated annually
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within specified timeframes prior to start
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start; includes testing for 12 specified drugs with MRO review

Professional Requirements

• Ascension Profile: Resume or professional profile submission required
• Ascension 2 Professional References: At least two qualifying references prior to first assignment
• Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment and updated annually
• Ascension EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service > 90 days
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start and updated annually
• Ascension Education Verification: Verification of relevant education within 90 days of start; rerun if break in service > 90 days
• Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions
• Ascension Employment Verification: Third-party verification for past seven years; updated annually or if break in service > 90 days

• Medical Imaging Contract Worker Form completed onsite by facility
• Post-assignment evaluation may be required
• Legal documentation of name change if applicable
