Job Description

• Perform physical therapy assessments and treatments

Required Experience

Required Certifications and Credentials

• BLS (Basic Life Support) – AHA or ARC only
• FMOLHS Skills Checklist – Unit specific, within 1 year prior to start, updated annually
• FMOLHS Core Competencies – Must be current within 1 year prior to start with passing score of 80% or higher; includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; renewed annually

Required Immunizations and Health Requirements

• MMR – Proof of 2 immunizations, positive titer, or declination accepted
• Fit Test – N95 mask fit test (3M 1860, 1860s, or 1870+), completed within 1 year prior to start, updated annually
• Hepatitis B – Proof of 3 immunizations, positive titer, or declination accepted
• Influenza Vaccine – Required for flu season (10/1 to 4/30); medical or religious exemptions accepted with proper documentation
• TB Test – Negative QuantiFERON blood test within 1 year prior or 2-step skin test; if positive, MD note or proof of treatment required
• Health Statement – Signed and dated within 1 year of start, stating ability to work without restrictions
• Varicella – Proof of 2 immunizations, positive titer, or declination accepted
• Tdap – Proof of vaccination within past 10 years or declination accepted
• Annual TB Questionnaire – Required within 6 months of start; if positive, documentation of evaluation and treatment completion required
• COVID-19 Vaccine – Optional (proof or exemption accepted)

Additional Required Testing and Documentation

• Criminal Background Check – Completed within 30 days prior to start; includes SSN address trace, county and national criminal checks, education and employment verification (last 5 years), driving record
• Drug Screen – Negative 10-panel drug screen within 30 days prior to start
• FMOLHS Competency Exam – Passing score of 80% or higher within 1 year prior to start
• FMOLHS Badge Photo – Forward facing, well-lit, neutral background, JPEG format
• FMOLHS Eye Exam & Color Vision – Ishihara Color Test and Snellen Vision test within 1 year prior to start
