Minimum Requirements 
<ul><li>Master's (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program.</li><li>Active Colorado Physical Therapy license required to start the assignment.</li><li>Home health experience with OASIS documentation required.</li><li>Experience performing comprehensive patient evaluations, developing measurable plans of care, and documenting skilled necessity for reimbursement.</li><li>Proven ability to provide skilled PT interventions in home settings, including therapeutic exercise, gait and balance training, transfer and stair training, and assistive device training.</li><li>Experience educating patients and caregivers on home exercise programs, fall prevention, energy conservation, and caregiver transfer techniques.</li><li>Experience coordinating with interdisciplinary teams and communicating with physicians regarding plan of care and discharge planning.</li><li>Daily travel to patient homes within an approximately 50-mile service radius. Ability to travel to multiple locations within a shift required.</li><li>Preferred: Experience with HCHB electronic documentation system.</li><li>Certifications: BLS from AHA or ARC may be required.</li></ul>

<ul><li>Facility Type: Home Health.</li><li>Shift Name: Days.</li><li>Shift Details: Days 0800-1600.</li><li>Work Days Per Week: Monday through Friday, five days per week. No call.</li><li>Travel Requirement: Daily travel to patient homes within the agency service area; may visit multiple patient locations per shift.</li><li>Dress Code: Scrubs in blue or black.</li><li>Interview Process: Phone or Microsoft Teams.</li><li>Education Requirement: Minimum Master’s degree.</li></ul>
