Interim Care Management Director
Overview
- Interim leadership role in Care Management and Resource Management.
- Start date: ASAP.
- Duration: 12 weeks.
- Schedule: Day shift, 8–5, 40 hours per week.
- The leader is expected to provide 24/7 leadership for the department, with hours that can flex to cover morning team meetings and afternoon needs.
Required Qualifications
- Bachelor’s degree in a relevant field, or 7+ years of direct experience in lieu of a bachelor’s degree.
- 3–5 years of experience in a closely related field with a bachelor’s degree.
- RN - Registered Nurse state licensure and/or compact state licensure.
- BCLS - Basic Life Support within 7 days of hire or transfer to position.
Preferred Qualifications
- Master’s degree.
- 3–5 years of previous leadership experience.
Role Responsibilities
- Oversees implementation and maintenance of a compliant Care Management program.
- Provides leadership and direction to staff.
- Conducts chart reviews for severity of illness and intensity of services.
- Refers cases not meeting criteria to the Physician Advisor or Utilization Management Committee.
- Identifies avoidable days and addresses them through communication systems.
- Coordinates discharge planning and social services functions.
- Serves as a liaison to community agencies for post-hospital placement and support.
- Ensures reporting of CPS and APS cases as required.
- Participates in Patient Care Conferences and Length of Stay meetings.
- May work in a staff role when required.
