Required Qualifications

Valid LMSW - Licensed Master Social Worker license in California
LMSW - Licensed Master Social Worker (2 years of experience)
LMSW - Licensed Master Social Worker experience within the last year
BLS issued by American Heart Association
Experience in a Hospital setting
2 years of recent discharge planning experience
Case Management experience

Role Summary

Coordinates care management, utilization review, discharge planning, and resource use
Supports patient assessment, care planning, implementation, and evaluation
Advocates for patients and supports continuity of care and cost effectiveness
