Licensed Practical Nurse (LPN) - Long Term Care

Facility & Department Overview

• Facility type: Skilled Nursing Facility (SNF) and Long Term Care (LTC)
• Census: Typically 80-95 residents; 20 rooms are not private
• Department: LTC | LPN
• Electronic Medical Record (EMR) system: PCC
• Patient Ratios:

  • RN: LTC 1:25, SNF 1:13
  • LPN: LTC 1:25, SNF 1:13
  • CNA: 1:10-15 (aim to keep around 10)

Job Duties

• Administer IV antibiotics and fluids (no titratable drips)
• Perform patient assessments, medication administration, dressing changes, and wound vac care
• CNAs handle vitals and activities of daily living (no blood sugars)
• Admission and discharge processes completed by RN
• Participate in interdisciplinary team rounds and meetings (encouraged)

Patient Population & Common Diagnoses

• Geriatric patients in LTC and rehab settings
• Common conditions: feeding tubes, IV antibiotics, post-surgical care, rehabilitation, chronic LTC needs
• Respiratory therapy available during the day Monday-Friday; RT support for AVAP/CPAP/BIPAP mostly at night

Schedule & Work Environment

• Shift hours: 5:00 AM to 5:00 PM
• Weekend and holiday work: Every other weekend and holiday required
• No call or standby requirements
• Floating: May float between units but generally kept on the same unit during a stretch
• Scrub color: Any professional scrub color allowed
• Orientation: One 12-hour shift on the floor; additional support available if needed
• COVID unit: Intermixed with other patients
• Respiratory protection: Use of both PAPR and N95 masks

Required Qualifications

• Valid LPN/LVN license in Illinois
• Minimum 1 year of Long Term Care nursing experience within the last year
• BLS (Basic Life Support) certification
• Driver’s license

• Pharmacy: On-site 24/7 pharmacy with Omnicell and medication carts for daily use
• Physician coverage: Hospitalist coverage after hours; Nurse Practitioner available weekdays; some private MDs follow patients; 24/7 on-call provider available
• Interdisciplinary team includes PT/OT/SLP, respiratory therapy, social workers, wound care nurse, dietician, and MDS nurses

This position offers a structured LTC environment with a focus on geriatric care, IV therapy, and interdisciplinary collaboration. Ideal for LPNs with at least one year of experience seeking a 13-week contract in a skilled nursing and long-term care setting.
