Job Opportunity: Home Health Physical Therapist

Location: Newport, Vermont
Employment Type: Contract
Contract Length: 13 weeks

Job Description

• Provide outpatient home health physical therapy services.
• Must have experience with OASIS, home health, and hospice care.
• Ability to travel from patient to patient for home visits.
• Must be able to manage and adjust to changes in schedule.
• Work schedule is Monday through Friday, 8 AM to 5 PM, with holiday coverage as needed.
• Use of an electronic medical record (EMR) system is required.

Required Qualifications

• Valid Physical Therapist license in Vermont
• Physical Therapist with at least 2 years of experience, including experience within the last year
• Experience in a Home Health setting
• Experience with OASIS
• Experience in hospice care
• Experience traveling patient to patient for home visits

Additional Information

• Prior travel experience is not explicitly required.
• Specialty experience required: 2 years in Rehabilitation Therapy.
