Diabetes Care Registered Nurse (RN)

Job Summary
The Diabetes Care RN plays a key role in managing and educating patients with diabetes, providing personalized care in collaboration with the diabetes care team. This includes medication management, device troubleshooting, patient education, and coordination with healthcare professionals.
Responsibilities
- Counsel patients on diabetes medications, including administration techniques, side effects, and outcomes.
- Prepare prior authorizations and appeal medication denials to ensure timely patient access.
- Collaborate with pharmacies and endocrinologists to resolve medication coverage issues and provide safe substitutions.
- Educate patients on diabetes fundamentals, biological and practical management, and culturally sensitive nutritional counseling (e.g., Mediterranean diet).
- Conduct glucose tolerance tests ensuring patient safety and accurate results.
- Provide exercise counseling tailored to individual patient needs and limitations.
- Instruct and troubleshoot continuous glucose monitors (CGMs) and insulin pumps, including device integration with mobile technology and data sharing.
- Upload patient home monitoring data for clinical team review during visits.
- Teach insulin administration, management of blood sugar levels, sick day rules, and glucagon pen usage.
- Coordinate with pump distributors for authorization and provide technical and emotional support.
- Ensure patients have adequate supplies to avoid interruptions in care.
- Respond to medical inquiries related to diabetes management.
- Collaborate with the diabetes care team, including depression care managers, dieticians, and health coaches, to provide comprehensive patient care.
Schedule
- Monday, Wednesday, Friday: 8:30 AM – 5:00 PM
- Tuesday & Thursday: Sessions end at 7:00 PM (at least one evening per week required)
- One to two Saturdays per month: 8:00 AM – 4:30 PM
- Coverage includes two weekends monthly and some evening sessions
Required Qualifications
- Valid Registered Nurse (RN) license in New York State
- Bachelor of Science in Nursing (BSN)
- At least one year of experience as a Diabetes Educator, with experience within the last year
- Certification or demonstrated expertise in Diabetes Education
- Experience with Epic electronic health records system
Contract Length
- 13 weeks
