Job Opportunity: Physical Therapist (PT) - Home Health (Days)

Job Description

• Provide physical therapy services to home health clients.
• Deliver direct patient care and collaborate with the healthcare team.
• Participate in developing comprehensive patient care plans.
• Use rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, and ADL equipment.
• Modalities application as needed.
• Must be Home Health Certified.

Requirements

• Specialty: Physical Therapist (PT)
• Experience:

  • Minimum 2+ years of acute nursing care (hospital-based) and home care experience.
  • Certified home health experience including proficiency with OASIS is required and non-negotiable.
  • Acute care experience preferred.
• Must be able to work independently with minimal supervision.
• Valid Driver’s License required.
• Must provide:

  • Work history and skills checklist relevant to physical therapy (updated within 1 year).
  • One supervisory signed reference from within the past two years.
  • Massachusetts PT license (must be uploaded with profile).
  • Active American Heart Association BLS certification.

Additional Information

• Prior travel experience is not explicitly required.
• Candidates residing within 50 miles of the facility will not be considered.
• Requested time off during the assignment must be noted and verified at profile upload; no more than 5 days of time off will be accepted.
• At least two specific interview dates/times must be provided; "anytime" availability is not accepted.
• Orientation includes 8 hours of non-billable time.
• Holiday and overtime multipliers apply at 1.25%.
• Call, call-back, and charge responsibilities are not applicable for this position.
