Outpatient Physical Therapist

Location: Cottonwood, Arizona
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

• Shift: Monday to Friday, either 5 days x 8 hours or 4 days x 10 hours
• Daily Patient Load: Approximately 11 patients
• Weekend Requirement: As needed
• On Call: No call required
• Floating: May be asked to float to surrounding outpatient clinics
• Guaranteed Hours: 40 hours per week (facility may call off up to 6 times during the 13-week assignment)
• First-time travelers: Accepted
• Locals: Not accepted without unit manager approval

Specialty & Experience

• Specialty: Therapy/Rehabilitation - Outpatient Physical Therapy
• Required Experience: Minimum 1 year of experience in specialty
• Prior Travel Experience: Not required

Required Licenses & Certifications

• Valid Arizona Physical Therapist license or Compact license required upon submitting
• BLS (Basic Life Support) certification (American Heart Association) required

Clinic & Work Environment

• Hospital-based ambulatory clinic supporting Orthopedics, Geriatrics, Cardiopulmonary, Neurological, Lymphedema, and Pelvic Health
• Facility is a short-term acute care hospital with teaching status and Level IV trauma designation
• Business casual dress code
• Electronic charting system used
• Free parking available

Required Documentation & Compliance

• Background check within 30 days of start (7-year history, includes criminal and sex offender searches)
• Drug screening (10-panel) within 30 days of start and updated annually
• Physical exam within 1 year of start, confirming ability to perform essential job functions
• Immunizations and titers required or declinations accepted for:

  • Tetanus/Diphtheria/Tdap
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Negative TB test or chest x-ray within 1 year of start, updated annually
• COVID vaccination series or declination with facility-specific form
• Seasonal flu vaccination or declination form required for candidates starting 09/29/2025 or later
• Fit mask (N95) required within 1 year of start and updated annually (not required for outpatient PTs but facility willing to complete onsite if needed)
• Completion of competency exams and medication/pharmacology competency within 1 year of start
• Security and confidentiality agreements must be signed prior to start
• Employment application with 3 years of work history and references required

Additional Notes

• No blocked scheduling
• Unit manager approval required for return-to-office or local hire exceptions
• Candidates must adhere to facility quality and patient safety standards, including medication administration protocols
• Photo for badge required (clear headshot with plain background preferred)
